What we are already doing

1.1.4 Reduce the number of older people needing emergency admissions to acute hospitals and reduce delayed discharges

Already we have...

1. Provided low level preventative services working in partnership with intermediate care and will continue to do so.

2. Re-ablement service now in place (Southborne) giving short term intensive support to develop/refine independet living skills.

We will...

1. Achieve the fuel poverty target as set out in the Local Area Agreement – tackle fuel poverty (Age-Friendly Strategy).

2. Provide reablement opportunities for people by giving short term intensive support to develop/refine independent living skills.

3. Increase early intervention e.g. by making GP’s & hospitals more easily accessible.

(Planning & Transport)

4.  Increase alternatives that divert people from hospital – re-ablement project, intermediate care.

(Adult & Community Support)

5.  Use opportunities to increase access to grant funded services, develop and commission new services and implement new services as part of stretch target in LAA1 - Increase the number of people benefiting from grant funded services targeted at older people aged 60 and over.
